Mencap – the voice of learning disability

'Mencap aim to improve the lives of people with a learning disability and their families now, and fight alongside them for a better future. They deliver support and advice services to people across England, Wales and Northern Ireland as well as working alongside a network of over 300 local groups.

The Community Enablement Team

Kirklees Council's Community Enablement Team is a service for adults and young people with learning disabilities and/or autistic spectrum conditions.

 

West Yorkshire Health Carer Partnership Live Well

Living well means looking after your health before anything goes wrong, as well as seeking help when something doesn’t seem right, illness strikes or you need to go into hospital. These resources are designed to help people take care of themselves or for their carers and the professionals who look after them.

 

 

PCPLD - The Palliative Care Network for People with Learning Disabilities

The PCPLD Network brings together service providers, people with a learning disability and carers working for the benefit of individuals with learning disabilities who have palliative care needs. Their ethos is that people with a learning disability should receive the coordinated support needed in living their lives to the full, until the end of their life.

 

Second Chance Headway

The Second Chance Headway Centre provides a therapy led day service for adults who are living with acquired or traumatic brain injury.

 

Safe Places

The Kirklees Safe Places scheme encourages independence and builds confidence for people over 16 with a learning disability.

A Safe Place is there for anyone vulnerable person who needs support when they go out and about.

 

Easy Health

With over 370 health resources that include pictures and simple text, Easy Health's information is accessible to anyone who finds written information hard to understand, including people with learning disabilities.
